Referral Criteria

Who

Anyone can refer to NorthCare Hospice & Palliative Care by calling our referral number (816) 691-5387 and talk with our Intake Coordinator or the main office number at (816) 691-5119. Referrals can come from physician offices, discharge planners, family members, and friends.

If you or someone from your family calls, we will call your doctor to discuss the illness and verify that Hospice is medically appropriate. The patient actually becomes admitted to Hospice by signing a consent form electing Hospice care.

When

Call NorthCare Hospice & Palliative Care as soon as a physician’s prognosis indicates that the patient has a life-limiting illness. The intake coordinator will be glad to speak with you about your specific situation and explain how NorthCare Hospice & Palliative Care can help. NorthCare Hospice & Palliative Care also offers a courtesy evaluation visit.

Our service area includes the following nine counties in Missouri and Kansas: Cass, Clay, Clinton, Jackson, Johnson, Leavenworth, Platte, Ray and Wyandotte.

Financial

Medicare, Medicaid and most private insurance companies cover the cost of hospice and palliative care services. As a non-profit agency, NorthCare Hospice & Palliative Care provides services regardless of a patient’s ability to pay.
